Welcome to the CCSS Report Card Template Builder!

This FREE on-line tool will assist in creating a Common Core State Standards aligned report card template for grades kindergarten through high school. Currently all grade levels are available for English language arts. Grades kindergarten through 8th grade mathematics and Algebra are also available. Users will be able to produce a MS Word document that can be edited and customized. Please note that documents created in the tool are not automatically saved. Users will need to save each grade level report card template to their desktop or other memory saving devise.

Before getting started you need to have determined the following:

  • At what levels do you want to report English language arts progress?
    EXAMPLE
    Claim Level: Reading: Students can read closely and analytically to comprehend a range of increasingly complex literary and informational texts.
    Strand Level: Reading Standards for Literature
    Substrand Level: Key Ideas and Details
  • At what levels do you want to report mathematics progress?
    EXAMPLE
    Domain Level: Measurement and Data
    Standard Level: Convert like measurement units within a given measurement system.
  • Do you want to report mathematics progress as a BUNDLE?
    A BUNDLE will include the claim, corresponding content standards, and mathematical practices.
    EXAMPLE
    Claim #1 – Concepts & Procedures
    “Students can explain and apply mathematical concepts and interpret and carry out mathematical procedures with precision and fluency.”.
    Practice Standards Content Standards
    1: Persevere
    6: Precision
    7: Seeing Structure
    8: Generalizing
    Write and interpret numerical expressions. (OA)
    Analyze patterns and relationships. (OA)
    Understand the place value system. (NBT)
    Convert like measurement units within a given measurement system. (MD)
    Represent and interpret data. (MD)
    Geometric measurement: understand concepts of volume and relate volume to multiplication and to addition. (MD)
    Classify two-dimensional figures into categories based on their properties. (G)
  • What progress indicators will you use?
    (Grades, 5 point scale, 4 point scale, 3 point scale)
  • How many progress periods will you have?
    (Quarters, Trimesters, Semesters)
  • Do you want to include boxes for :
    Science    Social Studies  Visual & Performing Arts
    Physical Education  Work Habits  Assessment Results
